DuBois Continuum of Care Community, Inc.


Our community's foundation was formed in the late 1970's when the DuBois Nursing Home opened its doors as a not-for-profit organization chartered to meet the region's need for quality long-term care. In 1996, the nursing home's board of directors created DuBois Villagean assisted living communityas an independent organization located on the same 13-acre campus. In 2000 and 2001, the nursing home added Memory Lane, a secure Alzheimer's/Dementia area, and Village View residential living apartments, respectively.

To better serve residents' individual needs, the two entitiesDuBois Nursing Home and DuBois Villagemerged in 2003 and established the DuBois Continuum of Care Community, Inc. With this seamless continuum of services, residents can tailor their lifestyle as needed without having to move to another community for extended care.

Throughout the years, we have maintained an excellent reputation for comfortable living and quality health care services. As a non-profit organization, we will promote an enriched community life with an emphasis on individualism and choice with excellence in care and services. We are commited to providing premier health and living services based on the needs of our region. We will provide a healthy work environment for employees that fosters personal and professional growth and well being.
